Job summary

TFI Family Services is hiring a full-time Aftercare Case Manager in Kansas to support families through permanency outcomes such as reintegration, adoption, guardianship, or aging out of care.

You will focus on risk management, provide crisis intervention, coordinate services with community agencies, and maintain thorough documentation while meeting families in person to support reunification and stability.

Qualifications

• Bachelor’s degree in human services required.
• Valid driver’s license required.
• Willingness to learn and training provided.

A Day in the Life

As An Aftercare Case Manager, You’ll Support Children And Families Who Have Achieved Permanency Through Reintegration, Adoption, Custodianship/guardianship, Or Aging Out Of Care. You’ll

• Focus on risk management and help families maintain stability during the aftercare period.
• Provide crisis intervention and work with public and private agencies offering supportive services.
• Maintain accurate documentation and reports.
• Coordinate community and educational services for families.
• Meet with families in-person to support reunification and resolve any concerns.
