Job summary

TFI Family Services in Chanute, KS is hiring a full-time Aftercare Case Manager to support children and families who have achieved permanency through reintegration, adoption, guardianship, or aging out of care. You’ll focus on risk management and help families maintain stability during the aftercare period.

You’ll provide crisis intervention, coordinate with public and private agencies, maintain accurate documentation, and meet with families in-person to support reunification and resolve

Qualifications

• A bachelor’s degree in human services.
• A valid driver’s license.
• A willingness to learn—we’ll provide all the training you need!
