Job summary

TFI Family Services is hiring a full-time Aftercare Case Manager to support children and families who have achieved permanency through reintegration, adoption, custodianship/guardianship, or aging out of care.

You will focus on risk management, provide crisis intervention, maintain records, coordinate services, and meet with families in-person to support reunification and resolve concerns.

Qualifications

• Bachelor’s degree in human services is required.
• Valid driver’s license is required.
• Willingness to learn; training provided.

A Day in the Life

As an Aftercare Case Manager, you’ll support children and families who have achieved permanency through reintegration, adoption, custodianship/guardianship, or aging out of care. You’ll

• Maintain accurate documentation and reports.


• Coordinate community and educational services for families.


• Meet with families in-person to support reunification and resolve any concerns.
