Job summary

Interim HealthCare of the Twin Cities in Shoreview, MN is seeking an on-call RN to triage and visit patients in their homes after hours. The role follows a 7 days on/7 days off schedule, with on-call hours typically from 5pm to 8am on weekdays and 8am to 8am on weekends.

Required are MN RN licensure, active license in good standing, and CPR certification. Minimum 12 months RN experience in the last five years, with hospice/home care experience preferred, and a valid driver’s license.

Responsibilities

• Be available to take calls and triage patients after normal business hours.
• Provide in-home visits for patients as needed.
• Operate on a full-time block schedule (7 days on/7 days off).
• Triages and coordinates care for patients remotely during after-hours periods.

Home Care After Hours On Call Triage Visit RN

We are looking for someone to be available to be On Call outside of normal business hours to take calls and triage patients, but also be available to make a visit if there is a need to our patients in their home setting.

Hours:

Full-time | There will be a block schedule of 7 days on and 7 days off. This position is considered full-time and will b e on-call outside of normal business hours,generally from 5pm - 8am, Monday - Friday and weekends from 8am - 8am.

A few must-haves for this position:

• Licensed to practice as a RN in the state of MN. License is active and in good standing.
• Minimum of twelve (12) months of accumulated experience as a registered nurse within the last five (5) years or, if no experience within the last five (5) years, evidence of current (within the last 12 months) clinical knowledge as deemed appropriate by the DHCS.
• One (1) year of hospice or home careexperience preferred.
• Able to travel within geographic area serviced by the office.Valid driver's license, reliable transportation, and proof of auto insurance.
