Job summary

Heartland Hospice is seeking an After Hours / On-Call RN to respond to patient needs during evenings, nights, weekends, and holidays. You will provide direct care, coordinate with physicians and the hospice team, and document visits in EMR while ensuring continuity of care.

Requirements include an active RN license in Missouri, CPR certification, and 1+ year of nursing experience. Hospice/home health experience is preferred, with a focus on compassionate, urgent-care support in the patient’s

Qualifications

• Current RN license in the state of employment.
• 1+ years of nursing experience.
• Hospice/home health experience preferred.
• CPR certification required or maintained.

Responsibilities

• Provide direct patient care and support after normal business hours.
• Serve as liaison between patients, families, physicians, and the hospice team.
• Respond promptly to urgent calls and symptom management needs.
• Document visits and communications accurately in the medical record.
• Participate in quality assessment and performance improvement programs.
• Ensure continuity of care with the daytime team.
