Job summary

Heartland Hospice is seeking an After Hours/On-Call Registered Nurse in Missouri to respond to patient needs during evenings, nights, weekends and holidays. You will provide direct patient care, coordinate with families and physicians, and document care through the EMR system.

Ideal candidates have an active RN license in Missouri, 1+ year of nursing experience, CPR certification, and reliable transportation for home visits.

Qualifications

• Current RN license in Missouri or eligibility to obtain
• 1+ years nursing experience
• CPR certification and valid driver’s license
• Compassionate, calm and dependable in urgent care situations

Responsibilities

• Provide direct patient care after hours and weekends
• Serve as liaison between patients, families, physicians and hospice team
• Document visits and communications accurately
• Participate in quality assessment and performance improvement programs
• Maintain continuity of care with daytime team
