Job summary

Heartland Hospice in Missouri is seeking an After Hours Registered Nurse to respond to patient needs during evenings, nights, weekends, and holidays, delivering compassionate care in patients' homes or facilities. You will provide direct patient care, document visits, and coordinate with the daytime team to ensure seamless care plans.

Requires active RN license, CPR certification, and 1+ year of nursing experience.

Qualifications

• Current RN license in the state of employment.
• 1+ years of nursing experience
• Hospice/home health care experience preferred
• CPR certification

Responsibilities

• Respond to patient needs after normal business hours and provide direct patient care
• Coordinate with the daytime care team and other staff to ensure continuity of care
• Document visits, plans of care, and communications accurately
• Participate in quality assessment and performance improvement programs
• Maintain infection control and safety standards during visits
• Possess strong clinical judgment and compassion in urgent care situations
