Job summary

Heartland Hospice is seeking an After Hours / On-Call Registered Nurse to respond to patient needs during evenings, nights, weekends, and holidays. You’ll serve as the primary liaison among patients, families, physicians, and the hospice team while delivering compassionate, high-quality care in the patient’s home or care setting.

The role includes documentation, coordination with the interdisciplinary group, and participation in QA/QI programs.

Qualifications

• Current RN license in the state of employment (or eligibility to obtain)
• 1+ years of nursing experience
• Compassionate, calm, and dependable in urgent care situations
• Excellent clinical judgment and ability to provide support in emotionally intense settings
• Comfortable providing care in patients’ homes, facilities, or wherever they reside

Responsibilities

• Respond to patient needs during evenings, nights, weekends, and holidays
• Serve as primary liaison between patients, families, physicians, and hospice team
• Document clinical visits and communications accurately
• Participate in quality assessment and performance improvement programs
• Work independently while ensuring continuity of care and coordination with the daytime care team
• Maintain Plan of Care in collaboration with the physician and interdisciplinary team
