Current RN licensure in the state is required; an associate degree is preferred. The team emphasizes compassionate, patient-centered hospice care and collaboration within an interdisciplinary group.

Qualifikationen

• Current RN licensure in the applicable state is required; associate degree preferred.
• Must participate in on-call rotation and maintain reliable transportation and valid driver's license.
• Strong communication and organizational skills; proficient in Microsoft Office Suite and EMR systems.
• Ability to perform physical tasks, including lifting patients over 200 lbs with assistance.

Aufgaben

• Conduct comprehensive nursing assessments and collaborate with IDG to develop care plans.
• Maintain timely and accurate documentation and coordinate care resources.
• Provide education and emotional support to patients and caregivers; participate in on-call rotations and admissions.
• Coordinate all aspects of patient care as Care Manager including scheduling and medication management.
• As On-call, respond to triage calls and assist with visits and care transitions.
