Job summary

AccentCare, Inc. is seeking an After Hours Registered Nurse (RN) for Hospice in Middlebury, CT. This full-time, on-site role operates 7 on/7 off from 5pm to 8am, serving New Haven and Fairfield County with a $90,000–$105,000 annual salary.

The position requires active RN licensure, an ADN or higher, and experience in hospice or related settings. Strong communication, coordination, and EMR skills are essential, with on-call duties included.

Qualifications

• Current RN licensure in the applicable state is required.
• Associate degree or higher preferred; CPR certification recommended.
• Proficiency in Microsoft Office Suite and EMR systems; strong communication skills.

Responsibilities

• Conducts comprehensive nursing assessments and collaborates with patients, families, physicians, and the IDG to develop, implement, and evaluate individualized plans of care.
• Maintains timely and accurate documentation and coordinates care with the care team.
• Provides education and emotional support to patients and caregivers; participates in on-call rotations, admissions and care transitions.
• If serving as a Care Manager, coordinates all aspects of patient care including scheduling, medication management, plan of care oversight, IDG collaboration, and supervision of staff.
• If serving in On-Call capacity, responds to triage calls, performs visits, coordinates care with the Administrator On Call, and may attend IDG meetings and family conferences.
