Job summary

AccentCare Inc. in Middlebury, CT is seeking an After Hours Registered Nurse (RN), Hospice for a full-time, on-site role. The schedule is 7 on / 7 off, 5pm–8am, with service coverage in New Haven and Fairfield County. Salary ranges from $90,000 to $105,000 per year.

You will perform comprehensive nursing assessments, coordinate with the interdisciplinary group, provide education and emotional support, and participate in on-call rotations and care transitions.

Qualifications

• RN license in applicable state is required.
• CPR certification is recommended/required for direct patient care.
• Hospice/home health or geriatrics/pediatrics experience preferred.
• Willingness to participate in on-call rotation and maintain transportation.
• Strong communication, organization, and teamwork skills.

Responsibilities

• Conduct nursing assessments and develop care plans with IDG.
• Maintain documentation and coordinate resources for complex cases.
• Educate patients and caregivers and provide emotional support.
• Coordinate care as Care Manager or On-Call, including meetings.
• Respond to triage calls and participate in care transitions.
