AccentCare, Inc. is seeking an After Hours Registered Nurse / RN, Hospice in Middlebury, CT.

This full-time on-site role offers a 7 on/7 off schedule from 5pm to 8am, with coverage across New Haven and Fairfield County and a salary range of $90,000–$105,000 per year. You will conduct comprehensive nursing assessments, coordinate patient care with the IDG, manage on‑call rotations, and support families with education and emotional care.

Qualifications

• Active RN licensure in the applicable state and ADN preferred.
• CPR certification recommended; hospice/home health experience valued.
• Willingness to participate in on‑call rotations and maintain reliable transportation.
• Strong communication and organizational skills; proficient with EMR systems.

Responsibilities

• Conduct comprehensive nursing assessments and collaborate with patients, families, physicians, and the IDG to plan and evaluate care.
• Maintain timely documentation and coordinate care to optimize resources, especially in complex cases.
• Provide education and emotional support to patients and caregivers, including on‑call duties and care transitions.
• Coordinate care if acting as Care Manager, including scheduling, medication management, and supervision of staff.
• Respond to triage calls and participate in IDG meetings and family conferences as needed.
