Qualifications

• 1 year hospice experience preferred but not required.
• Participate in field visits daily and office meetings weekly.
• Ability to adapt to patient’s status and needs on an on-call basis.

• Triage patient calls and make patient visits.
• Perform tuck-ins and handle admissions as needed.
