AdvisaCare Hospice is seeking a Full-Time Hospice RN to provide compassionate, in-home care and support to patients and families in Troy, NY. This role emphasizes presence, dignity, and personalized care, with on-call rotation and collaborative team involvement.

The successful candidate will hold an active RN license, have hospice experience, and demonstrate empathy and adaptability in a patient-centered setting.

Qualifikationen

• Current Michigan License in good standing.

• Participate in field visits daily, weekly office meetings as indicated.

Full-Time Case Management

As a Hospice RN with AdvisaCare Hospice, you'll provide emotional support and compassionate clinical care, helping patients experience their final days with dignity, comfort, and respect.

• Conduct appropriate supervisory visits
• Provide clinical observation and feedback to assigned team members
• Participate in interdisciplinary group meetings
• Help develop, review, and revise patient/family plans of care
• Advocate for the needs and wishes of patients and their families

• Initial and on-going assessment of patient's physical, functional, psychosocial and environmental needs
• Implement the individualized plan of care and revisions as needed
• Compliance with AdvisaCare Hospice Policies and Procedures, Medicare compliance
• Consulting with and educating the patient/family on disease process, self-care techniques, end-of-life care, dealing with issues of ethical concern
• Supervision of paraprofessionals providing services to patient
• Preparing clinical and progress notes, participating in IDG conferences
• Participate in Hospice performance improvement program
• Maintain high level of customer satisfaction with our clients, families and caregivers
• Communication of information using current process and technology within organization

Requirements:

• Current Michigan License in good standing
