AdvisaCare is hiring a dedicated Hospice RN to provide one-on-one home-based care in Michigan's Bay City, Midland and Saginaw tri-city area. You will assess patients, develop and revise care plans, and support families with compassion to help patients live their final days with dignity.

The role requires an active Michigan RN license, at least one year of hospice experience, and a commitment to full-time on-call scheduling.

Qualifications

• Active Michigan RN license in good standing.
• Minimum 1 year hospice experience.
• Patient- and family-centered approach.

Responsibilities

• Provide one-on-one home-based care to patients.
• Develop and revise individualized care plans with the team.
• Assess physical, functional, psychosocial, and environmental needs.
• Educate patients and families on disease process and self-care.
