VIA Health Partners in Sumter, SC seeks an experienced Registered Nurse to provide high-quality hospice and palliative care in patient and family settings.

The role requires active RN licensure, CPR, and strong patient education skills, with CHPN preferred. You will coordinate care, perform assessments, conduct home visits, and liaise with the multidisciplinary team to ensure compassionate, compliant care.

Competenze

• Diploma or Associate Degree in nursing required; Bachelor's degree preferred.
• Current RN license in good standing.

Mansioni

• Coordinate patient and caregiver care across the care continuum.
• Perform nursing assessments to guide management of patient/caregiver needs.
• Make home visits and provide interventions per the plan of care.
• Follow and adjust the plan of care as goals change.
• Provide telephone or home-visit triage and timely interventions.
• Liaise with physicians, facilities, and the interdisciplinary team.
• Document care, education and medication teaching in records.
• Ensure infection control and safety standards are met.
• Maintain professional relationships with external agencies.
