Job summary

Qualifications

• Diploma or Associate Degree in nursing required; Bachelor’s Degree preferred.
• Current, active, and unrestricted RN license required.
• CPR certification required; CHPN preferred.

Responsibilities

• Coordinate patient/caregiver care and conduct nursing assessments.
• Perform visits and follow the plan of care; update as goals change.
• Maintain liaison with healthcare providers to coordinate multidisciplinary care.
• Provide patient education and document care in records.
