Job summary

Prisma Health Hospice is seeking a dedicated registered nurse to provide compassionate end-of-life care. You will perform comprehensive nursing assessments, implement and adjust the plan of care, educate families, and collaborate with the care team to meet patient goals.

Minimum of an Associate Degree in Nursing with RN licensure; one year of hospice or home care experience preferred. BSN is preferred for some roles. This position is on-site within the Upstate SC hospice program.

Qualifications

• Associate degree in Nursing and RN license required.
• One year RN experience; hospice or home care preferred.
• BSN may be preferred or required by department.

Responsibilities

• Provide hospice and palliative nursing care per Plan of Care.
• Assess patient and family needs; update interdisciplinary care plan.
• Document assessments and care plans in medical record.
• Coordinate with interdisciplinary team to ensure quality care.
• Participate in bereavement support and family education.
