Job summary

Heartland Hospice is seeking an After Hours/On-Call Registered Nurse in Missouri to respond to patient needs during evenings, nights, weekends, and holidays, delivering compassionate care when it is most needed. You will work with the daytime team to maintain continuity of care.

As an on-call RN, you report to the Administrator or Patient Care Manager, provide direct patient care after hours, coordinate with families, physicians, and the hospice team, document visits, and participate in quality

Qualifications

• Current RN license in the state of requested employment (or eligibility to obtain).
• 1+ years of nursing experience.
• A hospice heart: compassionate, calm, and dependable in urgent care situations.
• Excellent clinical judgment and the ability to provide support in emotionally intense settings.
• Comfortable providing care in patients' homes, facilities, or wherever they reside.
• Valid driver's license, automobile insurance, and reliable transportation.
• Current CPR certification.

Responsibilities

• Respond to patient needs during evenings, nights, weekends, and holidays.
• Provide direct patient care and support after normal business hours.
• Serve as primary liaison between patients, families, physicians, and the hospice team.
• Evaluate patient and family needs to guide clinical decisions and implement the plan of care.
• Document clinical visits and communications in the EMR.
• Participate in Quality Assessment and Performance Improvement programs.
• Work independently while coordinating with the daytime care team.
