Job summary

Heartland Hospice seeks an After Hours/On-Call Registered Nurse to provide compassionate in-home and facility-based care during evenings, nights, weekends, and holidays. You will act as the liaison between patients, families, physicians, and the hospice team, responding to urgent needs and ensuring a seamless plan of care.

The role emphasizes accurate documentation, adherence to the hospice plan, and participation in quality improvement.

Qualifications

• Current RN license in the state of employment.
• 1+ years of nursing experience.
• Compassionate, calm, and dependable in urgent care situations.
• Excellent clinical judgment and the ability to provide support in emotionally intense settings.
• Comfortable providing care in patients’ homes, facilities, or wherever they reside.
• CPR certification is current.

Responsibilities

• Respond to patient needs during evenings, nights, weekends, and holidays and provide direct patient care after hours.
• Serve as the primary liaison between patients, families, physicians, and the hospice team during shifts.
• Respond promptly to urgent calls, patient visits, and symptom management needs.
• Evaluate patient and family needs to guide clinical decisions and implement the plan of care.
• Document clinical visits and communications accurately and maintain medical records.
• Collaborate with the interdisciplinary group and participate in quality improvement programs.
