Job summary

Luminary Hospice in Ohio is seeking an on-call registered nurse to support patients and families after hours in their homes. You will perform assessments, develop and adjust care plans, administer medications, and coordinate with the interdisciplinary team to meet evolving needs.

The role requires an active nursing license, CPR certification, and 2–5 years of relevant hospice or end-of-life care experience; Medicare Hospice knowledge is preferred.

Qualifications

• Bachelor's or Associate's degree in nursing required.
• Maintains an active nursing license in good standing.
• Maintains an active CPR certification.
• 2–5 years of experience in healthcare management, hospice, or palliative care preferred.
• Experience with Medicare Hospice Certification and CHAP guidelines preferred.
• Knowledge of hospice care services and interdisciplinary collaboration.

Responsibilities

• Completes initial and comprehensive patient and family assessments to determine hospice needs.
• Provides professional nursing care using the nursing process and data to determine diagnoses.
• Assesses status and writes plan of care; revises plan as needed after hours.
• Develops and revises care plans with patient/family involvement.
• Initiates preventive and rehabilitative nursing procedures and administers medications as prescribed.
• Assists with activities of daily living and supports patient self-sufficiency and comfort.
• Coordinates patient care with Case Managers after hours.
• Maintains accurate clinical notes and documents pain/symptom changes.
• Communicates with physicians regarding patient needs and orders.
• Teaches self-care techniques; provides health counseling to patients and families.
• Collaborates with the hospice interdisciplinary team to address emotional needs.
• Participates in quality improvement activities as needed.
• Performs additional duties as assigned.
