Job summary

Luminary Hospice is seeking an after-hours Registered Nurse to support patients in their homes across rotating shifts (Mon-Thurs 5:00 PM–8:30 AM and Fri 5:00 PM–Mon 8:30 AM). The role emphasizes compassionate, patient- and family-centered care with coordination by the interdisciplinary team.

A 1-week-on, 1-week-off schedule helps balance workload and life. Responsibilities include comprehensive assessments, care planning, medication administration, documentation, and ongoing patient education,

Qualifications

• Bachelor's or Associate's degree in nursing (required).
• Active nursing license in good standing.
• Active CPR certification.
• 2–5 years experience in healthcare management, hospice, or palliative care (preferred).
• 1 year Medicare Hospice Certification and CHAP guidelines experience (preferred).
• Knowledge of hospice care and interdisciplinary team procedures.
• Proficient in MS Office and able to learn software systems.
• Strong organizational, verbal and written communication, analytical, and detail-oriented skills.

Responsibilities

• Complete initial and comprehensive patient and family assessment to determine hospice needs.
• Provide professional nursing care and utilize nursing process for diagnosis.
• Assess patient status and revise care plans as needed after hours.
• Develop care plans and involve patient and family in planning.
• Administer medications and treatments per physician's plan.
• Assist patients with activities of daily living and promote self-sufficiency.
• Coordinate after-hours patient care with Case Managers and team.
• Document clinical notes and track pain/symptom changes.
• Communicate with physicians and follow orders.
• Educate patient and family on self-care and health counseling.
• Collaborate with family and hospice team to support emotional needs.
• Participate in quality improvement initiatives as needed.
• Perform other duties as assigned.
