Strong interpersonal and communication skills with good judgement.
Must hold a valid driver’s license and have access to a vehicle.

Responsibilities

Provide case management, skills training and support in ADL, behavioral plans, and community integration.
Develop, implement and monitor person-centered ISP goals with individuals.
Assess behaviors and complete daily data, monthly progress notes and quarterly reviews.
Assist individuals with obtaining services and entitlements (housing, benefits, etc.).
Foster community integration and support individuals to actualize this goal.
Participate in weekly staff, supervision and clinical meetings.
