Qualifications

  • Bachelor’s Degree or High School Diploma with two years’ relatable experience.
  • Strong interpersonal and communication skills with good judgement.
  • Must hold a valid driver’s license and have access to a vehicle.

Responsibilities

  • Provide case management, skills training and support in ADL, behavioral plans, and community integration.
  • Develop, implement and monitor person-centered ISP goals with individuals.
  • Assess behaviors and complete daily data, monthly progress notes and quarterly reviews.
  • Assist individuals with obtaining services and entitlements (housing, benefits, etc.).
  • Foster community integration and support individuals to actualize this goal.
  • Participate in weekly staff, supervision and clinical meetings.
