Advocates in Fitchburg, WI is hiring a case management and support role starting at $20.00/hour. You will work with individuals with developmental or mental health challenges to implement ISP plans, provide ADL support, and promote community integration.

A high school diploma or GED is required; bachelor’s degree or two years related experience is preferred. A valid driver’s license and reliable vehicle are required to transport clients.

Formación

• Provide case management, ADL support, and behavioral plan implementation.
• Develop, implement and monitor ISP and person-centered goals.
• Assist individuals with services and entitlements (housing, benefits, etc.).
• Participate in weekly staff meetings, supervision and clinical meetings.

Responsabilidades

• Provide case management, ADL support, and behavioral plan implementation.
• Develop, implement and monitor ISP goals with individuals.
• Assess behaviors and complete daily data, monthly progress notes, quarterly reviews.
• Assist individuals with housing, insurance benefits, and other entitlements.
• Foster community integration and support clients to actualize this goal.
• Participate in weekly staff meetings, supervision and clinical meetings.
