Responsibilities

  • Provide case management, skills training and support in ADL, health care, and community integration.
  • Develop, implement and monitor Individual Service Plans (ISP) and person-centered goals.
  • Document daily data, monthly progress notes and quarterly reviews.
  • Assist clients in obtaining services and entitlements (housing, benefits, etc.).
  • Foster community integration and support client independence.
  • Attend weekly staff and clinical meetings as scheduled.
