Job summary

Default Brand is seeking a Consumer Services Coordinator (Case Management) for San Bernardino, CA. The role focuses on coordinating services for individuals with developmental disabilities, drafting IPPs, and maintaining ongoing assessments and meetings with families and agencies.

The position requires travel across the San Bernardino Upper Desert region and adherence to Medicaid Waiver standards, with a focus on thorough documentation and advocacy.

Qualifications

• Bachelor’s degree from an accredited college or university and one year of experience in intellectual/developmental disabilities, social work, special education, early childhood development, or related field.
• Master’s degree in social work, psychology, public health nursing, teaching, special education, early childhood development, or related field from an accredited college or university.
• Bilingual preferred but not required.
• Ability to establish and maintain effective working relationships with others.
• Maintain good attendance and punctuality.
• Ability to follow oral and written direction.
• Good verbal and written communication skills.
• Full use of an automobile and a valid California driver’s license and insurance.
• Must have and maintain a safe driving record.

Responsibilities

• Coordinate case management services for consumers.
• Develop and implement IPP and addendums; prepare progress reports.
• Attend and prepare for required meetings (IEP, ITP, IHC, etc.).
• Advocate for consumers and families; connect to resources.
• Document and report incident and crisis information.
• Coordinate assessments across medical, educational, psychological, and vocational areas.
• Arrange placement in residential facilities or day programs as clinically indicated.
• Ensure Medicaid Waiver standards and timely documentation.
• Maintain required administrative records and CEU participation.
