Job summary

Work Creation Program in Santa Ana, CA, is seeking a Case Manager to support adults with developmental disabilities. You will coordinate services, develop individualized ISPs, and advocate for client independence in a collaborative, field-based setting.

The role requires a Bachelor's in Human Services, 2 years’ experience with vocational plans, and a valid CA driver’s license. Compensation includes full benefits, PTO/holidays, mileage reimbursement, and a 401(k) after 60 days.

Qualifications

• Bachelor's degree required; Human Services field preferred.
• Two years of experience writing and implementing vocational plans in a program setting preferred.
• Experience working with individuals with developmental disabilities.
• CPR and First Aid certification (must be completed within 30 days of hire).
• Clean criminal background check as outlined in DDS licensing requirements (LIC 508).
• Reliable vehicle with valid California driver's license and current auto insurance.
• Strong knowledge of behavior modification and human development.
• Excellent organizational skills and ability to work independently.
• Outstanding written and verbal communication skills in English (Spanish a plus).
• Demonstrated ability to work effectively on an interdisciplinary team.

Responsibilities

• Review and analyze assessment data to develop comprehensive ISPs.
• Coordinate and attend interdisciplinary team meetings.
• Prepare Semi-Annual Progress Reports and maintain accurate case records.
• Train and support direct care staff on goal implementation and documentation.
• Provide job coaching support and field coverage with clients as needed.
• Ensure all documentation meets confidentiality guidelines and regulatory standards.
• Monitor client progress and adjust services to meet evolving needs.
• Serve as a liaison between clients, families, and service providers.
