Qualifications

• Must have a High School diploma or GED or 1 year of relevant experience.
• 5 years of relevant experience required.
• Experience with Medi-Cal managed care (Cal MediConnect and Medicare) preferred.
• Bilingual Spanish speaking preferred.
• Bachelor's Degree in Social Work or equivalent preferred.

Responsibilities

• Provide telephonic social services support and resource referrals for Blue Shield Promise members.
• Coordinate with team members and internal departments to support member needs.
• Serve as the first line of telephonic response for incoming Social Services calls.
• Assess social service needs and coordinate support to improve member quality of care.
