Job summary

Blue Shield of CA is seeking an Advanced Social Service Representative to deliver telephonic social services support and resource referrals to Promise members, and to provide administrative support to the Social Services Management Team.

The role involves regular outreach to members and family members to assess needs, coordinate support, and improve quality of care, with collaboration across internal departments.

Qualifications

• Requires a High School diploma or GED or 1 year of additional relevant experience in lieu of a diploma.
• Requires 5 years of relevant experience.
• Demonstrated 2 years of experience with Medi-Cal managed care including Cal MediConnect and Medicare.
• Proficient skills with computer programs, including Microsoft Excel, Outlook, Word, and PowerPoint.
• Excellent communication skills both orally and in writing.
• Bilingual Spanish speaking preferred.

Responsibilities

• Provide telephonic social services support and resource referrals for Promise members.
• Coordinate with the Social Services Management Team and other internal departments.
• Conduct outreach to assess social service needs and coordinate supportive actions.
