Qualifications

• Requires 5 years of relevant experience
• Medi-Cal managed care knowledge including Cal MediConnect and Medicare
• Bachelor's Degree in Social Work or equivalent preferred
• Proficient with Microsoft Excel, Outlook, Word, and PowerPoint
• Excellent written and verbal communication skills
• Knowledge of health disparities in diverse communities
• Bilingual Spanish speaking preferred
• Experience coordinating with internal departments

Responsibilities

• Provide telephonic social services support and resource referrals for members
• Assess social service needs and coordinate support to improve quality of care
• Serve as first point of contact for Social Services department calls
• Coordinate with team members and other internal departments to meet member needs
• Provide administrative support to Social Services Management Team
