Job summary

LiveWell Homecare Agency seeks an advanced nurse to join an interdisciplinary team, promoting preventative care, conducting screenings, and coordinating disease management with primary care providers, patients, and families.

You will assess needs, develop individualized care plans, and facilitate communication to remove barriers, while supporting care transitions and ongoing education in a clinical setting.

Qualifications

• Two years of RN experience required, preferably in Med/Surg.
• Graduate from a specialty nursing program is required; a Bachelor’s Degree in Nursing is preferred.

Responsibilities

• Implement clinical interventions based on guidelines and coordinate tests.
• Document activities in electronic health records and manage patient data.
• Educate patients on disease management and support care transitions.
• Contribute to professional development and participate in care coordination programs.
• Assist in medication administration, IV monitoring, and specimen collection; advise on treatment options.
