Job summary

LiveWell Homecare Agency, LLC is seeking a qualified RN to provide individualized, outcome‑oriented nursing care using the nursing process: assessment, planning, and care delivery in alignment with agency policies and procedures.

You will administer and document medications, develop care plans, support patient and family needs, ensure safety and infection control, and participate in professional development while maintaining privacy and cultural sensitivity.

Qualifications

• Graduate of an accredited School of Nursing (preferred).
• RN experience in acute care preferred.
• Valid RN license and BLS certification; additional certifications may be required by department.

Responsibilities

• Administer, transcribe, and document medications following policies, including narcotic management and reconciliation.
• Develop, evaluating, and updating patient care plans, documenting outcomes, and assessing patient needs and comfort levels.
• Facilitating patient/family-centered care, effective communication, and discharge planning.
• Ensuring safety by verifying patient identity, infection control, and emergency protocols.
• Engaging in professional development, adhering to legal and ethical standards, and responding to feedback.
• Utilizing technology for documentation and communication, and maintaining compliance with policies.
• Supporting patient privacy, promoting a culture of safety, and respecting cultural and age-related differences.
