Job summary

You will support providers, patients, and families, aiding in diagnosis and treatment planning and maintaining thorough documentation.

Responsibilities include patient assessment, education on disease management, and coordination with healthcare teams to ensure guideline adherence.

Qualifications

• Minimum 2 years RN experience.
• Bachelor’s degree preferred.
• Strong clinical, communication, and collaborative skills.

Responsibilities

• Assess healthcare, educational, and psychosocial needs to develop individualized care plans.
• Facilitate communication among patients, families, and healthcare teams to overcome barriers and optimize treatment.
• Educate patients and families on disease management, care coordination, and health strategies.
• Implement clinical interventions based on risk assessment and evidence-based guidelines.
• Coordinate diagnostic testing and monitor therapy adjustments in collaboration with providers.
• Document activities accurately in electronic health records and collect relevant data.
• Participate in ongoing professional development and incorporate current best practices.
• Support post-discharge transitions and contribute to program design and evaluation.
• Perform clinical procedures such as IV insertion, medication administration, and specimen collection, within scope of practice.
