Job summary

Theoria Medical is seeking a part-time Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to support post-acute care and SNF residents in Columbia, MO. You will perform daily rounds, develop treatment plans, and collaborate with a multidisciplinary team to deliver high-quality, patient-centered care.

Qualifications

• Active U.S. license as APRN or PA and board certification is required.
• Experience in post-acute care or SNF settings preferred.
• Ability to perform rounds, assess residents, and manage treatment plans.

Responsibilities

• Provide high-quality, patient-centered care to SNF residents.
• Conduct daily rounding and develop/adjust treatment plans.
• Prescribe medications and coordinate with facility staff.
• Maintain on-site attendance for scheduled shifts and charting.
• Ensure compliance with clinical standards and regulations.
