Theoria Medical in Troy, OH is seeking a full-time Advanced Practice Provider to serve local skilled nursing facilities within a mission-driven care community.

You will deliver high-quality, patient-centered care, perform daily rounds, and manage treatment plans while coordinating with facility staff. A flexible, autonomous practice is supported by advanced technology and a collaborative clinical team.

Qualifications

• Active U.S. license and board certification by ANCC/AANPCB/PNCB/AACN (for NPs) or NCCPA (for PAs).
• Experience in geriatrics or skilled nursing facilities preferred.
• Eligible for employment in the United States.

Responsibilities

• Provide high-quality, patient-centered care to residents in skilled nursing facilities.
• Conduct daily rounding and address acute/emergent issues.
• Prescribe medications/treatment plans and adjust as needed.
• Communicate with facility nursing staff via phone or secure messaging.
• Maintain reliable on-site attendance for scheduled shifts.
