Intermountain Health is seeking an experienced Nurse Practitioner or Physician Assistant for the Heart Failure Clinic in Wheat Ridge, CO. You will partner with a certified Heart Failure Cardiologist to advance a premier program in Colorado.

The role includes outpatient clinic consults and inpatient rounding, with 4/10s, Monday–Friday, no weekends or on-call requirements. Requires CO licensure eligibility, BLS/ACLS, and 2+ years in heart failure cardiology.

Qualifications

• Graduate from an accredited Physician assistant or Nurse Practitioner program.
• Licensed as an NP or PA in CO or able to obtain.
• DEA and full prescriptive authority in CO or able to obtain.
• BLS and ACLS required.
• Minimum of 2 years of experience as an NP or PA in a Heart Failure Cardiology setting.

Responsibilities

• Practice at Wheat Ridge Cardiology clinic and Lutheran Hospital.
• Work outpatient clinic consults and follow ups; inpatient rounding and consults.
• Collaborate with Heart Failure Cardiologist to manage stage 3 and 4 heart failure patients.
• Provide medication management, dietary and lifestyle counseling, and regular follow ups.

About this role:

Intermountain Health is seeking an experienced Nurse Practitioner or Physician Assistant for our Heart Failure Clinic. The selected candidate will partner with the a certified Heart Failure Cardiologist to continue to build a pre-eminent Heart Failure Program in the state of Colorado.

• You would practice at our Wheat Ridge Cardiology clinic and Lutheran Hospital. Additional locations may be added with program growth
• Hours are4/10’s Mon-Fri, no call and no weekends. Your day off will vary
• You would work with stage 3 and 4 heart failure patients to ensure medication compliance, monitoring dietary and lifestyle changes, regular follow ups and much more
• This position would be part of the continuity of patient care from hospital to clinic and back to the primary care physician or cardiology service line
• You would work a combination of outpatient clinic consults and follow ups along with inpatient rounding and consults

The hourly range for this position is listed below. Actual hourly rate dependent upon experience.
