Job summary

American Family Care Sanford seeks an Advanced Practice Provider (APP) to care for patients within the scope of training and approved agreement, at our franchised location. The APP will collaborate with other providers and staff to maintain smooth patient flow and high satisfaction, with duties varying between franchises.

Responsibilities include comprehensive histories, medical assessments, ordering tests, and initiating therapies or referrals.

Responsibilities

• Take detailed health histories, review records, develop medical assessments and order appropriate tests.
• Formulate medical diagnoses and coordinate referrals to facilities and resources.
• Institute emergency measures and stabilize patients in critical situations.
• Interpret patient data to determine management and treatment.
• Educate patients and families about health care and promotion.
• Perform other duties as assigned.
