Job summary

American Family Care Morton Grove is seeking an Advanced Practice Provider (APP) to care for patients within approved training and agreement parameters. The APP will work collaboratively with other providers and staff to ensure efficient patient flow and high patient satisfaction at a franchised location.

Responsibilities include detailed health histories, diagnoses, and referrals, with emphasis on timely assessments, emergency stabilization when needed, and patient education.

Qualifications

• Excellent communicator with staff, patients, and family.
• Professional appearance and attitude.
• Active PA or NP license in Illinois.
• Able to multi-task and work independently.

Responsibilities

• Perform complete, detailed health histories and review patient records.
• Formulate medical or nursing diagnoses and arrange referrals as needed.
• Institute emergency measures and stabilization when required.
• Interpret patient data to guide treatment decisions.
• Provide health care guidance to patients and families.
• Other duties as assigned by the franchise.
