Job summary

American Family Care is seeking an Advanced Practice Provider (APP) to care for patients within the scope of training and approved agreement. The position involves collaborative work with other providers at a franchised AFC location to ensure efficient patient flow and high satisfaction.

Duties and benefits vary by franchise. The AFC model emphasizes walk-in, urgent care and primary care services across diverse locations, with onsite labs and imaging capabilities at many centers.

Qualifications

• Active and current Physician Assistant or Nurse Practitioner licensure in the state of practice, DEA number, and state-controlled substance certificate as required.

Responsibilities

• Perform complete medical histories, review patient records, assess and order appropriate tests.
• Formulate medical and nursing diagnoses and refer patients to appropriate facilities or resources.
• Institute emergency measures and stabilization in critical situations.
• Interpret and analyze patient data to guide management and treatment.
• Provide instructions on health care and prevention to patients and families.
• Other duties as assigned.
