Job summary

Theoria Medical is seeking a full-time Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to serve local skilled nursing facilities in Ovid, Michigan. You’ll provide patient-centered care, perform daily rounds, prescribe treatments, and collaborate with a multidisciplinary team to ensure high-quality care for vulnerable populations.

Qualifications

• Active U.S. license and board certification required (ANCC, AANPCB, PNCB, or AACN for NPs; NCCPA for PAs).
• Specialties include Internal Medicine, Family Medicine, Emergency Medicine, General Practice, Physical Medicine and Rehabilitation, and Geriatric Medicine.

Responsibilities

• Provide high-quality, patient-centered care to residents in skilled nursing facilities.
• Conduct daily rounding, prescribe medications and treatment plans, and address acute/emergent issues.
• Communicate with facility nursing staff via phone or secure messaging as needed.
• Maintain reliable on-site attendance for scheduled shifts.
• Ensure compliance with clinical standards, facility policies, and regulatory requirements.
