Theoria Medical invites an Advanced Practice Provider to join our team serving skilled nursing facilities in South Haven, Michigan. You will deliver patient-centered care, conduct rounds, and coordinate with facility staff to ensure quality outcomes.

Qualifications

• Active U.S. license and board certification in relevant field.
• Specialties include Internal Medicine, Family Medicine, Emergency Medicine, General Practice, PM&R, and Geriatric Medicine.
• Nurse Practitioners and Physician Assistants eligible for provider roles.

Responsibilities

• Provide high-quality, patient-centered care to residents in skilled nursing facilities.
• Conduct daily rounding and address acute/emergent issues.
• Prescribe medications and treatment plans.
• Communicate with facility nursing staff via phone or secure messaging.
• Maintain reliable on-site attendance for scheduled shifts.
