SSM Health is seeking an APP (FNP or PA) to join our growing Portage, WI family medicine practice. This 100% outpatient setting offers a schedule with Monday–Friday daytime hours, no nights or weekends, and potential for a 4-day work week. A dedicated MA and on-site RN support ensure collaborative care.

Your Qualifications:

• Graduate of a nationally accredited Physician Assistant (PA) or Nurse Practitioner (NP) Program
  • WI NPs must be APRNs
  • NPs must have Family certification
• Ability to obtain PA/NP licensure in WI
• Experience is a plus
