Job summary

SSM Health is seeking an APP (FNP or PA) to join our growing Portage, WI family medicine team. You will provide 100% outpatient care in a multi-disciplinary clinic, supported by on-site RN triage, s et-al, and a collaborative group of physicians and APPs.

The role offers a daytime Monday–Friday schedule with no nights, weekends, holidays or mandatory calls, plus potential for a 4-day work week and a structured admin time. Comprehensive benefits and a strong community focus complete the package.

Qualifications

• Graduate of a nationally accredited PA or NP program.
• WI APRN registration is required for NPs.
• NPs must have Family certification; PA/NP licensure in WI required.
• Experience is a plus.

Your Qualifications

• Graduate of a nationally accredited Physician Assistant (PA) or Nurse Practitioner (NP) Program

• WI NPs must be APRNs
• NPs must have Family certification

• Ability to obtain PA/NP licensure in WI
• Experience is a plus
