Qualifications

  • 3+ years of clinical experience in acute or primary care, with 2 years preferred in telehealth.
  • Expert-level business and clinical knowledge, able to work independently and collaboratively.

The Advanced Practice Provider (APP) I, CareLine role involves diagnostic patient care primarily through virtual consultations via video or phone. The incumbent acts as the dispositional authority for after-hours and weekend clinical calls, assessing, diagnosing, treating, and documenting patients' health status through data collection.

ESSENTIAL JOB DUTIES/RESPONSIBILITIES:

  • Assess clinical problems via virtual or phone consultations, perform physical evaluations, analyze patient data, develop and implement management plans, including health education and referrals.
  • Plan patient care considering cultural background, understanding, personality, and support systems, serving as a patient advocate.
  • Gather and analyze health data to determine maintenance and improvement strategies.
  • Communicate with the patient's PCP and other providers to report data and ensure guideline compliance.
  • Use Telemedicine to achieve optimal outcomes, collaborating with on-call PCPs and evaluating progress.
  • Enhance telehealth practices through data analysis and system improvements.
  • Communicate effectively using various tools and technologies.
  • Utilize resources to provide safe, effective, and cost-efficient services.
  • Deliver exceptional customer service and professionalism.
  • Participate in clinical rounds, meetings, and quality programs as needed.
  • Follow departmental goals, standards, regulations, and policies, practicing under a written or electronic practice agreement.
  • Contribute to the development of telehealth policies and participate in quality improvement activities.
  • Support growth through committee participation and provide feedback for patient care improvement.
  • Coordinate services with other programs and perform additional duties as assigned.
KNOWLEDGE, SKILLS, AND ABILITIES:
  • Travel to Georgia or Tennessee for onsite reviews as mandated by law, quarterly or bi-annual respectively.
  • Possess expert-level business and clinical knowledge, with the ability to work independently and collaboratively.
  • Demonstrate excellent clinical judgment, problem-solving, and communication skills.
  • Proficient in Microsoft Office Suite and capable of operating telemedicine systems compliant with regulations.
  • Willing to travel for meetings and trainings up to 10% of the time, including potential travel to other states for compliance reviews.
  • Fluent in English; bilingual skills (Spanish/Creole) are a plus.
  • Prepared to obtain independent practice authority in applicable states.
EDUCATION AND EXPERIENCE CRITERIA:
  • Bachelor’s degree in Nursing (BSN) and graduate of an accredited nursing school with certification in a specialty area; Master's degree in Nursing required.
  • Board certification by AANP or ANCC required for NPs.
  • Basic Life Support (BLS) certification required upon hire.
  • Current DEA registration may be required depending on state practice.
  • Multistate licensure, including licenses in at least two states from the list provided.
  • At least 3 years of clinical experience in acute or primary care, with 2 years preferred in telehealth.
