This role involves providing diagnostic patient care primarily through virtual consultations via video or phone. The provider serves as the authority for after-hours and weekend clinical calls, assessing, diagnosing, treating, and documenting patients' health status, including physical and psychosocial aspects.
Essential Job Duties/Responsibilities
- Assess acute and non-acute clinical problems via virtual means, perform physical evaluations, analyze patient data, and develop management plans including health education and referrals.
- Plan patient care considering cultural background, understanding, personality, and support systems, advocating for patients.
- Collect and analyze health data to determine maintenance or improvement strategies.
- Coordinate with the patient's primary care provider and other medical professionals to report health data and ensure guideline compliance.
- Use Telemedicine to optimize patient outcomes, collaborating with on-call PCPs and implementing protocols to support clinical goals.
- Contribute to quality improvement of telehealth practices through data analysis and system enhancements.
- Communicate effectively using various tools to build relationships and deliver care across the continuum.
- Utilize resources to ensure safe, effective, and cost-efficient services.
- Provide excellent customer service and professionalism to all stakeholders.
- Participate in clinical rounds, meetings, and quality management programs.
- Follow departmental standards, policies, and practice agreements, and participate in policy development.
- Engage in quality improvement activities and support organizational growth through committee participation.
- Offer feedback to improve patient care practices and coordinate services with other programs.
- Perform any additional duties as assigned by management.
Knowledge, Skills, and Abilities
- Adhere to state-specific travel requirements for onsite reviews (Georgia, Tennessee, etc.).
- Possess expert-level business and clinical knowledge, with the ability to work independently and collaboratively.
- Demonstrate excellent clinical judgment, problem-solving skills, and the ability to prioritize under pressure.
- Communicate professionally with team members and patients.
- Proficient in Microsoft Office Suite and capable of operating telemedicine systems.
- Willing to travel up to 10%, including to specific states for compliance reviews.
- Fluent in English; bilingual skills (Spanish/Creole) are a plus.
- Willing to obtain independent practice authority in applicable states.
Education and Experience
- Bachelor’s in Nursing (BSN) and graduate of an accredited nursing program with certification in a specialty; Master’s degree required.
- Board certification by AANP or ANCC.
- Current BLS certification from AHA or ARC.
- Multi-state licensure, including licenses in FL, VA, and at least two other states from the list provided.
- Minimum of 3 years’ clinical experience in acute or primary care, with telehealth experience preferred.
