Advanced Practice Provider, CareLine - PRN (Remote)

Role Overview: The APP in the CareLine is responsible for virtual diagnostic patient care through video or telephone consultations, providing on-call coverage for after-hours and weekends. The role involves assessing, diagnosing, treating, and documenting geriatric patients' health status, including physical and psychosocial aspects.

Essential Job Duties

• This is a PRN position requiring a minimum of 160 hours annually, with flexible scheduling including nights, weekends, and holidays.
• Assess acute and non-acute clinical problems remotely, perform evaluations, develop management plans, and document findings.
• Evaluate the need for immediate intervention, coordinate care, and serve as a patient advocate.
• Collaborate with primary care providers and other medical staff to ensure quality care and compliance.
• Lead efforts in telemedicine to achieve optimal outcomes, document care, and participate in quality improvement initiatives.
• Maintain professional communication, adhere to policies, and participate in meetings and training as required.

Knowledge, Skills, and Abilities

• Strong clinical judgment, problem-solving skills, and ability to work under pressure.
• Excellent communication and collaboration skills with multidisciplinary teams.
• Proficiency in Microsoft Office and telehealth technology.
• Ability to travel locally up to 10% and work flexible hours, including holidays.

Education and Experience

• Bachelor's in Nursing (BSN) and graduate of an accredited nursing school with certification in a specialty; Master's degree required.
• Active RN license, board certification (AANP or ANCC), and BLS certification.
• Experience in acute/primary care (minimum 3 years) and telehealth (preferred).
• Multi-state licensure including FL and at least two other states from the specified list.
