Qualifications

Job description

Advanced Practice Provider 1, CareLine (Remote) Telehealth

Join us at ChenMed as an Advanced Practice Provider 1, CareLine (Remote) Telehealth and be part of a transformative healthcare team.

Role Overview: The APP I is responsible for diagnosing and managing patient care primarily through virtual consultations via video or phone. The role includes assessing, diagnosing, treating, and documenting patients' physical and psychosocial health, and serving as the dispositional authority for after-hours and weekend clinical calls.

Key Responsibilities:
  • Assess acute and non-acute clinical issues remotely, perform physical evaluations, analyze patient data, and develop management plans.
  • Plan patient care considering cultural, psychological, and social factors, and serve as a patient advocate.
  • Collect and analyze health data to determine appropriate interventions.
  • Collaborate with primary care physicians and other healthcare providers to ensure compliance and optimal patient outcomes.
  • Utilize telemedicine tools effectively, support clinical outcomes, and participate in quality improvement activities.
  • Communicate professionally using various tools and technologies.
  • Adhere to departmental policies, standards, and regulatory requirements, practicing in accordance with practice agreements.
  • Contribute to the development of telehealth policies and participate in clinical and quality meetings.
  • Support growth initiatives and provide feedback to improve patient care.
Knowledge, Skills, and Abilities:
  • Adhere to state laws regarding travel for onsite observation (Georgia, Tennessee, etc.).
  • Demonstrate expert clinical judgment, problem-solving skills, and ability to work under pressure.
  • Effective communication and collaboration skills.
  • Proficiency in Microsoft Office Suite and telemedicine systems.
  • Willingness to travel as required, including to specific states for compliance purposes.
  • Fluent in English; bilingual skills are a plus.
  • Ability to obtain independent/autonomous practice as an NP in applicable states.
Qualifications:
  • Bachelor’s degree in Nursing (BSN) and graduate of an accredited nursing program, with certification in specialty area; Master’s degree in Nursing required.
  • Board certification by AANP or ANCC.
  • Current BLS certification.
  • Multi-state licensure, including licenses in FL, VA, and at least two other states from the specified list.
  • Minimum of 3 years' acute/primary care experience; telehealth experience preferred.
