The Advanced Practice Provider (APP) I, CareLine is responsible for diagnostic patient care primarily through virtual, remote consultation via video conference or telephone. The incumbent in this role serves as the dispositional authority for after-hours and weekend clinical calls. They are accountable for assessing, diagnosing, treating and precisely documenting patients' physical and psychosocial health status through the collection of health data.
ESSENTIAL JOB DUTIES/RESPONSIBILITIES:
- Assess acute and non-acute clinical problems via virtual video conference or telephone, perform and document physical evaluations and patient histories, analyze trends, and develop, document, and implement a patient management plan based on findings. Assist in developing care plans including health education, referrals, and counseling.
- Plan patient care considering cultural background, understanding level, personality, and support systems to anticipate and identify health issues. Act as a patient advocate.
- Collect and analyze comprehensive data related to patients' health needs to determine appropriate maintenance or improvement methods.
- Coordinate with the patient's PCP and other providers to report health data and ensure guideline compliance.
- Use Telemedicine to achieve optimal patient outcomes, collaborating with on-call PCPs as needed, and evaluating progress.
- Contribute to enhancing telehealth practices through data analysis and system improvements.
- Communicate effectively using various formats and technologies to build relationships and deliver care.
- Ensure services are safe, effective, and financially responsible.
- Provide excellent customer service and professionalism.
- Participate in clinical rounds, meetings, and quality management programs.
- Adhere to departmental goals, standards, policies, and practices, including practice agreements.
- Contribute to the development of telehealth policies and protocols.
- Engage in quality improvement activities and support growth initiatives.
- Offer feedback to improve patient care and coordinate services with other programs.
- Perform additional duties as assigned by management.
KNOWLEDGE, SKILLS AND ABILITIES:
- Adhere to state laws regarding travel for onsite reviews (Georgia, Tennessee, or others as applicable).
- Possess expert-level business and clinical knowledge, with the ability to work independently and collaboratively.
- Achieve clinical performance metrics consistently.
- Conduct telemedicine visits compliant with regulations.
- Demonstrate excellent clinical judgment, problem-solving, and prioritization skills.
- Communicate professionally with team members and patients.
- Operate effectively within a multidisciplinary team.
- Proficient in Microsoft Office Suite and other relevant systems.
- Willing to travel up to 10% for meetings and training, including quarterly visits to Georgia or other states as required.
- Fluent in English; bilingual skills (Spanish/Creole) are a plus.
- Willing to obtain independent practice authority in applicable states and maintain expertise in the field.
EDUCATION AND EXPERIENCE CRITERIA:
- Bachelor’s degree in Nursing (BSN) and graduate of an accredited nursing program with specialty certification; Master’s degree in Nursing required.
- Board certification by AANP or ANCC required for NPs.
- BLS certification from the American Heart Association or Red Cross required upon hire.
- Current DEA registration for Schedule II-V controlled substances may be required.
- Multi-state licensure, including licenses in at least two states from the list (FL, VA, GA, MI, MO, OH, PA, TN, TX, IL, KY, LA).
- Minimum 3 years of acute/primary care clinical experience; 2 years telehealth experience preferred.
