Job summary

Millennium Physician Group is seeking a Nurse Practitioner for General/Primary Care in our ambulatory practice. The NP will provide comprehensive, evidence-based primary care across the lifespan, evaluating illnesses, managing chronic diseases, and delivering preventive services in collaboration with a physician-led team.

You will operate with minimal supervision within the APP scope, perform office-based procedures, document encounters in the EHR, educate patients, and support quality

Qualifications

• Master's-level clinical training required for APP scope.
• Independent practice within APP collaborating model.
• Ability to diagnose and manage routine to moderately complex cases.

Responsibilities

• Evaluate and manage acute and chronic conditions across lifespan.
• Develop individualized treatment plans including meds and preventive services.
• Perform office-based procedures within scope.
• Order and interpret labs and imaging as needed.
• Educate patients and caregivers on diagnoses and self-management.
• Document encounters accurately in the EHR and maintain regulatory compliance.
• Collaborate with physicians and care teams to coordinate patient care.
