Job summary

Qualifications

• Master's-level clinical training applied within the APP scope.
• Demonstrates ability to collaborate with physicians and care teams.

Responsibilities

• Evaluate, diagnose, and manage patients with acute illnesses, chronic diseases, and preventive care.
• Develop treatment plans including medication management, immunizations, and screenings.
• Manage common primary care conditions (hypertension, diabetes, hyperlipidemia, etc.).
• Order, perform, and interpret laboratory tests and imaging as needed.
• Perform office-based procedures aligned with scope of practice.
• Conduct annual wellness visits and preventive health examinations.
• Coordinate referrals to specialty care and supportive services.
• Educate patients and caregivers on diagnoses, treatments, and self-management.
• Document encounters and outcomes in the electronic health record.
• Participate in quality improvement and population health initiatives.
